MI Choice Medicaid Waiver Program

It is less costly and a preferred alternative to nursing home care, ensuring people get the support they need in their own homes or setting of their choice.

MI Choice services can be provided in:

  • Your own home
  • The home of a caregiver or family member
  • A residential setting such as an assisted living facility, adult foster care home or home for the aged

MI Choice has specific income, asset, and level of care requirements.

To find out if you are eligible, please call AAANM and ask to speak with one of our Access and Eligibility Specialists at 1-800-442-1713.  You will be asked a series of health-related questions to help determine if you qualify for an in-home assessment. If you qualify for an assessment, you will either, be referred to the scheduler or placed on a waiting list if the program is at capacity.

A Supports Coordinator (Nurse and/or Social Worker) will conduct an in-home assessment to determine your eligibility for the program.  If you are eligible, they will also work with you to develop a plan that will allow you to live as independently as possible. The Supports Coordinator will help you monitor your plan and make changes to your plan as your needs change.

Eligibility

The MI Choice Medicaid Waiver program is funded by the Department of Community Health and Human Services (MDHHS). This program provides a wide range of home and community-based services coordinated by a Supports Coordinator to ensure participants are able to remain as independent, healthy, and safe as possible within their own community.

You may be eligible for MI Choice if you are:

  • 65 years of age or older
  • A disabled person aged 18 or older
  • Live in Antrim, Benzie, Charlevoix, Emmet, Grand Traverse, Kalkaska, Leelanau, Manistee, Missaukee, or Wexford County.
  • Your income does not exceed $2,829 a month and your personal assets do not exceed $2,000 a month, excluding your residence and one vehicle
  • You have multiple medical and service needs
  • You need the MI Choice program to prevent having to move into a nursing home

  • Homemaking
  • Personal Care
  • Personal Emergency Response Units
  • Private Duty Nursing
  • Respite Care
  • and more
