Job Opening: Care Manager Nurse

*This is a work from home position with regional travel and some in-person/office based meetings in Traverse City. Candidate must live in Northwest Michigan.

Job Summary: Care Managers use person-centered principles to assess the complex medical, functional, psychological, financial, and environmental needs of care management participants. The Care Manager will work with the participant and their supports to develop a person-centered plan and arrange for needed services and supports, advocate for participants and their allies, and ensure the health, safety, and welfare of program participants. Aspects of this position will be conducted as part of a nurse/social work team.

Pay: Based on experience

Schedule: Monday through Friday 8am-4:30pm with some flexibility in work schedule. Position is remote based, however it does require travel throughout the northwest Michigan 10-county service area and may include occasional trips to other parts of the State; Ability to work outside scheduled hours when need arise.
