Assists older adults and persons with disabilities, along with their families and caregivers, by providing information about supports and resources for aging, caregiving needs, and long-term care planning during critical times. They help individuals and their families navigate the complex landscape of long-term care options by assessing needs, offering information about available services and community-based long-term care options, and guiding them towards the best choice based on their preferences and financial situation. This empowers them to make informed decisions about their long-term care plan.

Care Transitions Specialist
Provides person-centered care transition coordination and support to older adults transitioning from hospitals and other medical care facilities back to their homes and communities. The CTS serves as a trusted liaison between participants, health care providers, community-based organizations, social service agencies, and other partners to improve access to services, support successful transitions of care, and reduce avoidable hospital and emergency department utilization.

Care Transitions Program Coordinator
Responsible for the day-to-day coordination, implementation, and ongoing operation of the CTCS Program. The Coordinator provides program-level oversight while also delivering direct Community Health Worker and care transition services to older adults transitioning from medical care facilities back to their homes and communities. The Transition Specialist provides proactive discharge planning, transition coaching, service coordination, participant education, and post-discharge follow-up. The position supports participants in identifying and addressing health-related and social needs that may affect their ability to safely remain in the community.
